Screening Guide
The information collected on this form will be held in confidence. This form is the first step in assessing your program needs. Please provide as much detailed information as possible. Once complete and submitted, a BFW staff member will contact you within 24 hours.  Have a question or need assistance; we’re here to help. Program Information

Building Foundations for Women is an 8-week life skills and personal development program.  BFW supports women experiencing poverty and other life challenges to build stable and active lives and move toward their employment and educational goals.

BFW is designed to assist individuals with personal development/ management, transition planning, goal setting and post program support.  Referrals to external support services, mental health/ addiction treatment, employment services and income support programs will be provided as needed.

In the course of the calendar year, BFW offers two 8-week in class sessions, 4 days/per week, 9:30am – 2:30pm, and case management by appointment. TTC tokens, stipends and lunch will be provided.  In addition, a 6 month post program follow up period will follow with various support activities.

Intake Process:  Prior to acceptance into the program, Ve’ahavta will determine eligibility through the following activities:

Step #1     
Telephone/ Online Screening Form:  Individual screening will be conducted to determine client eligibility and interest in participating in the program. To be eligible OW clients must obtain and bring their OW Referral Form to the Information session. 

Step #2 
Information Session:  Eligible applicants will be invited to attend a session where detailed information regarding the pre-requisites and program details will be described. Applicants will have an opportunity to ask questions and determine if the program is the right course for them. (Approx. 30 mins.)  

Step #3
Intake Questionnaire and Interview:  Ve’ahavta will administer a short intake questionnaire to determine client needs and conduct an individual one-on- one interview. At this stage, only selected applicants will be asked to continue in the intake process. (Approx. 20 mins.) 

Step #4
Group Session:  Qualified selected applicants will be invited to participate in a group session at a date closer to the start of the program to evaluate the individual’s ability to engage in a group setting. (Approx. 90 mins.) 

Next Steps:  Applicants that have been identified to start the program will be notified by telephone after the group session and formally accepted. All applicants who are NOT accepted into the program will be notified and will be provided with information regarding other programs and services available in the community.
